Job Summary:
Reporting to the Director of Care management, the Nurse Care Management Educator has responsibility for the onboarding and training of all Nurse Care Managers in their designated workflows. The educator will work closely with the Director and the Program Managers to ensure onboarding is completed and any educational needs are addressed within the existing team.
Duties/Responsibilities:
- Works with Director and Program Managers to ensure that an onboarding schedule is prepared for the new colleague.
- Guide new clinical colleagues through training and mentoring
- Participate in RN and NCM training development
- Collaborates with key stakeholders to identify opportunities to improve utilization management education. Works with Program Managers to develop, implement, and monitor the effectiveness of performance improvement activities.
- Maintains current knowledge of state regulations for nursing, and CCMs as it relates to scope of practice for Nurse Care Managers.
- Identifies and provides appropriate resources to educate staff members on clinical conditions and processes.
- Ensures that processes, policies, and procedures are adhered to. Serve as a resource for staff to guide to appropriate resources
Required Skills/Abilities:
- Strong knowledge of nurse care management functions, including transitions of care, chronic case management, disease management, reimbursement practices, and quality improvement initiatives
- Thorough, in-depth knowledge of nurse care management processes, legal rules and regulations and best practices in healthcare
- Skilled in observation, training and evaluating colleague performance
- Ability to effectively leverage business and organizational knowledge within and across functional areas
- Must possess a high degree of emotional intelligence and integrity, driven and focused work ethic
- Continuous desire to learn and embrace new methods; ability to adapt and be resilient.
- Self-starter with the ability to think creatively and work effectively
- Ability to build a relationship and work effectively with various seniorities and diverse populations.
- Excellent critical reasoning, decision-making, and problem-solving skills to analyze situations, determine risks, and find solutions to prevent future issues and resolve recurring defects
- Willingness and ability to travel, up to 20%
Education and Experience:
- AA/AS degree in Nursing required; BA/BS degree in Nursing (BSN) or Healthcare Administration preferred
- A valid, active Registered Nurse (RN) license in state(s) of employment required
- A minimum of 5 years', current direct nurse care management required
- A minimum of 2 years' experience as a Nursing manager/supervisor or in a similar leadership capacity preferred
- Work in a community-based clinic, public health department or specialization with the senior population preferred
- A passion for mentorship and team-development, particularly across multiple functional areas
- A problem-solving orientation and eagerness to identify process gaps and implement practical solutions in a matrixed organization
- Proficient PC skills
- Fluency in Spanish or other languages spoken by people in the communities we serve is desirable, but not required
