Summary

Job Description:

Baptist Health is looking for Nurse Call Center RN - Part-Time to join their team. This is a remote work position that requires residency in KY or IN.

The Discharge Call Nurse is responsible for promoting optimal clinical outcomes in the acute care setting byoverseeing the discharge follow-up call process. This role involves contacting patients within 24-48 hours post discharge to assess satisfaction, evaluate compliance with discharge instructions, and identify trends for qualityimprovement. The Discharge Call Nurse will ensure a high standard of patient care while supporting theorganization's mission and values.

Essential Duties

  • Make telephone contacts with patients within 24-48 hours after discharge to assess patient education,satisfaction, and compliance with discharge instructions (75% of time).

  • Utilize a centralized database to record and track patient responses, ensuring accurate documentation

  • Intervene promptly when issues are reported after discharge, utilizing the grievance process and notifyingRisk Management as necessary.

  • Analyze patient feedback to make recommendations for process and performance improvements withinthe department (5% of time).

  • Identify trends in patient responses that may indicate areas requiring enhancement in quality of care orhospital processes (5% of time).

  • Collaborate with management and clinical staff to implement changes based on patient feedback and bestpractices.

  • Serve as a patient advocate by anticipating and addressing customer needs, prioritizing high-qualitycustomer service.

  • Communicate effectively with patients, staff, and other stakeholders, ensuring clarity and empathy in allinteractions.

  • Employ independent judgment to resolve complaints and ensure compliance with CMS standards.

Minimum Education, Experience, Training and Licensures Required

  • Registered Nurse (BSN or higher preferred) with 5 years' experience.

  • RN license required

24 hours weekly; 3 days a week
