Job description

Description

Become familiar with the facility's administrative structure as introduced in the pre-training orientation session. Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. Report all accidents and incidents observed during your shift. Use only authorized abbreviations established by this facility when recording information. Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical. Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. Agree not to disclose residents' protected health information and promptly report suspected or known violations of such disclosure to the Administrator. Report any known or suspected unauthorized attempts to access the facility's information system.

Perform only those nursing care procedures that you have been trained to do. Ensure the resident's room is ready for receiving the resident (e.g., bed made, name tags up, admission kit available). Greet residents and escort them to their room. Introduce residents to their roommates and other residents and personnel as appropriate. Inventory and mark residents' personal possessions as instructed. Store residents' clothing. Assist residents with packing their possessions during transfers or discharges. Transport residents to new rooms or to the receiving area. Assist with loading/unloading residents to/from vehicles as necessary. Participate in and receive the nursing report as instructed.

Follow established policies concerning exposure to blood/body fluids. Make beds as instructed. Put extra covers on beds as requested. Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Weigh and measure residents as instructed. Ensure residents unable to call for help are checked frequently. Answer resident calls promptly. Check residents routinely to ensure their personal care needs are met.

Assist residents with food arrangements (e.g., informing visually impaired residents about their tray contents). Record residents' food/fluid intake. Report changes in eating habits to the Nurse Supervisor/Charge Nurse. Keep water pitchers clean and filled with fresh water on each shift, within easy reach. Perform after-meal care (remove trays, clean hands, face, clothing). Perform all tasks according to established policies and procedures, and as instructed by supervisors. Follow work schedules and cooperate with interdepartmental personnel to ensure adequate nursing services. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per facility policies. Maintain confidentiality of all resident care information, including protected health information. Report any incidents of unauthorized disclosure. Report all allegations of resident abuse or property misappropriation. Respect residents' refusal of treatment and report such requests to your supervisor.

Supervisory Requirements: This position has no supervisory responsibilities.

Qualifications: Must possess at least an 8th-grade education. Ability to read, analyze, interpret business and technical documents, and communicate effectively. Ability to write reports and respond to questions. Mathematical skills include applying fractions, percentages, ratios, and proportions. Reasoning skills include solving practical problems and interpreting instructions in various forms.

Physical Demands: Occasionally lift/move up to 25 pounds. Prolonged use of a computer. Regularly sit, stand, walk, talk, read, or hear. Use of office equipment. May assist in evacuations during emergencies.

Work Environment: Typically low to moderate noise level. Reasonable accommodations available for disabilities.
