Description

Definition and Basic Function: Provides oversight of comprehensive, holistic health care that will include direct care to individuals in keeping with Jessica and Friends Community overall mission. Initiates services designed to promote and maintain the health, safety and general well-being of the individuals that we serve.

Responsibilities

• Works collaboratively with residential managers to ensure that medical appointments are completed as well as all follow-up care from the appointments. This includes documentation on all scheduling tools (appointment chart and Google calendar) and that the residential manager has all needed documentation for medical appointments.
• Oversees the medication administration testing and observation of medication administration for renewal certifications.
• Oversees utilization of Health Risk Safety Tool (HRST)
• Complete HRST on all new individuals
• Schedule yearly HRST team meetings for all individuals in residential
• Review HRST monthly making needed revisions

4. Participates in House/Program Manager meetings quarterly to facilitate training on health-related issues.

• Supports direct support staff when dealing with illnesses, injuries, or health related issues involving the individuals we serve
• Completes weekly medication checks at each location
• Monitor medication labels against the MARs
• Check for expired medications
• Check that bubble packs are initialed
• Ensure that the MARs are completed accurately without errors
• Works closely with managers and staff when individuals are hospitalized and ensure follow up after discharge.
• Completes monthly checks on medical equipment and supplies (AED, first aid supplies, over the counter medications, etc.) to ensure they are functional
• Promotes and supports healthy choices to advance and maintain wellness (i.e., nutritional menus; monitoring weight; exercise)
• Participates in Safety Committee meetings.
• Responsible for organizing and responding to messages from WellSpan patient portal.
• Facilitates health-related trainings with managers and staff.
• Serves as emergency on-call for urgent medical issues of individuals.
• CPR/First Aid training and recertification of staff.
• Assumes all other duties as assigned.

Summary

Job Requirements: Registered Nurse, licensed in Pennsylvania, with valid driver's license. Experience in home health and/or human services preferred. Additional training/experience in working with individuals with Intellectual Disabilities/Autism a plus.

Work Schedule: Full-time, generally 40 hours per week to be determined by the needs of the individuals. May involve limited weekend/evening work if special needs arise (which will be offset by time off during the week).
