Job description

St. Luke's is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our highly collaborative Rheumatology team and support our growing Bone Health Program based in Meridian, Idaho. The selected candidate will join an established practice of 3 APPs and 3 physicians specializing in bone health, dedicated to delivering exceptional patient-centered care. Bone Health is a strategic, value-based initiative focused on improving the diagnosis, treatment, and long-term management of osteoporosis and other metabolic bone diseases. This innovative program partners closely with Orthopedic Surgery, Spine Care, Primary Care, Endocrinology, Women's Services, and Physical Therapy to provide comprehensive patient education and promote early intervention. Our goal is to improve both the quality and quantity of life for patients through proactive bone health management.

This role embraces team-based care and innovative care delivery models, utilizing virtual care and technology-enabled solutions to improve patient access, engagement, and continuity of care. The ideal candidate will be comfortable leveraging digital tools and working collaboratively across specialties to deliver efficient, coordinated care.

In addition to supporting the Bone Health Program, this position will spend up to three days per week embedded within the St. Luke's Spine Clinic, partnering closely with Spine providers to strengthen integration between Spine Health and Bone Health services and enhance the patient experience. This is an exciting opportunity to join a growing program making a meaningful impact across the health system while working alongside a supportive, collegial team committed to innovation and clinical excellence.

What You Can Expect:

  • Experience in Primary Care, Orthopedics, Spine Care, Endocrinology, Rheumatology, or other related specialties strongly preferred.

  • Interest in virtual care, population health, and technology-enabled patient management.

  • Demonstrated ability to work effectively in a multidisciplinary, team-based care environment.

  • Malpractice insurance as well as malpractice tail coverage

  • $3,500 Professional Development Fund
