Job summary

UNM Hospital Sandia Vista Clinic is seeking a primary care clinician to join a multidisciplinary team serving patients of all ages in a newly built clinic in Southwest Albuquerque. Qualifications

• No minimum experience required
• Bilingual English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo in applicable area

Responsibilities

• Adhere to the Patient Centered Medical Home core expectations.
• Assess, diagnose and develop treatment plans within scope of practice protocols.
• Provide education for patients on preventive care and medical treatments.
• Document patient care accurately in the medical records and communicate with the health care team.
• Collaborate with medical/nursing/counseling staff to provide comprehensive care and ensure continuity of care.
