Job summary

Sharp Rees-Stealy Medical Group in San Diego is seeking a Family Nurse Practitioner or Physician Assistant for 1.0 FTE in the Sharp Extended Care and Care at Home programs. The role provides float coverage across outpatient services, with a focus on post-acute and home-based care.

The candidate must hold an active CA NP/PA license, board certification, and demonstrate independent practice and strong communication for goals-of-care discussions with patients and families.

Qualifications

• Minimum of 2 years of full-time NP/PA outpatient experience.
• Active, unrestricted California NP or PA license.
• Current board certification in applicable specialty.
• Must be comfortable working independently.
• Excellent communication skills with the ability to lead goals-of-care and advanced care planning discussions with patients and families.

Responsibilities

• 100% outpatient position providing float coverage across Sharp Extended Care and Care at Home services.
• Lead advanced care planning conversations and collaborate with patients, families, and care teams.
• Post-acute care management in SNFs including patient rounding, care coordination, and discharge planning.
• Conduct approximately 5–6 home visits per day delivering intermittent skilled medical care.
• No weekends / no inpatient care.
