Job summary

LMH Health is seeking a Nurse Practitioner (APRN) to provide direct care under established policies and protocols in Kansas. The role coordinates care for patients by applying decision-making skills based on patient needs and priorities.

The APRN will develop therapeutic relationships, collaborate with the clinical team, coordinate discharge planning, and contribute to governance and best practices within the unit. Regular attendance is essential.

Qualifications

Master’s degree in Nursing and active Kansas APRN license.
BLS certification for Healthcare Providers is required.
APRN license to practice in Kansas is required.
Prior clinic experience is preferred.

Responsibilities

Develop therapeutic relationships with patients and families based on medical history, symptoms, examinations, diagnoses and treatments.
Coordinate care and communicate with the clinical team through documentation to optimize patient outcomes.
Assess and coordinate patient discharge planning needs with the healthcare team.
Lead inter-professional coordination of patient-centered care and governance activities.
Perform other duties within the APRN scope and maintain regular attendance.
