Job summary

Halcyon Home in Texas is seeking a dedicated Home Health Physical Therapist (Full Time PV) to support our geriatric patients in Tarrant County and surrounding areas. You will assess needs, design plans of care, and collaborate with patients, families, and the care team to promote independence and safety at home.

Qualifications

• Current PT license.
• Experience in home health or related healthcare setting preferred.
• Ability to work independently and manage time effectively.

Responsibilities

• Performs ongoing clinical assessment, including determining patient needs and plan of care.
• Develops and evaluates the plan of care in partnership with patient and caregivers.
• Maintains required continuing education and licensing requirements.
• Instructs patients and families on transfer techniques and equipment use, ensuring safety.
• Provides patient and caregiver education and counseling.
• Applies infection control concepts and standard precautions in patient care.
• Documents clinical notes and adjusts the plan of care based on assessments.
• Communicates with physicians and other health practitioners involved in the plan of care.
• Accepts assignments within education and competence; supervises PT assistants and home health aides as appropriate.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders as referred.

Key Responsibilities:

• Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Meets mandatory continuing education requirements of the Agency and licensing board.
• Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
• Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
• Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
• Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
• Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients’ needs.
• Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
• Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.

Requirements

• Current PT license
• Knowledge or experience utilizing Homecare Homebase EMR would be a plus
• Experience in home health or related healthcare setting preferred
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively.
