Halcyon Home in Fort Worth, TX is seeking a dedicated Home Health Physical Therapist (Full Time PV) to support our geriatric population across Tarrant County and surrounding areas. You will perform ongoing clinical assessments, develop and evaluate the plan of care, and collaborate with patients, families, and physicians.

Qualifications

• Current PT license in good standing
• Experience in home health or related healthcare setting preferred
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively

Responsibilities

• Performs ongoing clinical assessment, including OASIS assessments, to identify patient needs and level of functioning
• Develops and evaluates the plan of care with patient and caregiver(s)
• Maintains required continuing education per Agency and licensing board
• Instructs patients and families on transfer techniques and equipment use
• Provides patient and caregiver education and counseling
• Performs infection control and standard precautions in care delivery
• Documents clinical notes, orders, and updates the plan of care
• Communicates with physicians and other health care practitioners
• Accepts assignments aligned with education and competence
• Oversees PT assistants and home health aides as appropriate
• Plans/treats neuromuscular, neurological, and orthopedic conditions per physician orders

Key Responsibilities:

• Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
• Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Meets mandatory continuing education requirements of the Agency and licensing board.
• Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
• Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
• Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
• Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
• Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients’ needs.
• Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
• Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.

Requirements

• Current PT license
• Knowledge or experience utilizing Homecare Homebase EMR would be a plus
• Experience in home health or related healthcare setting preferred
• Strong clinical and assessment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively.
