Job summary

UChange Home Care Services in Illinois seeks Certified Nursing Assistants (CNAs) to provide compassionate at-home care to clients in the 60419 and 60411 service areas. This role offers flexible scheduling with part-time and full-time opportunities based on client needs, and competitive pay based on experience.

As a CNA, you will assist with personal care, daily living activities, mobility, meal preparation, companionship, and EVV documentation while following established care plans and agency

Qualifications

• Illinois CNA certification required.
• Healthcare/home health or clinical experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently in a client's home.
• Reliable attendance and punctuality.
• Valid drivers license and automobile insurance when driving.
• Reliable smartphone for EVV and documentation.
• Ability to pass background checks and other pre-employment requirements.

Responsibilities

• Assist with bathing, grooming, dressing, and toileting.
• Provide mobility support, positioning, and transfers.
• Assist with activities of daily living and meal preparation.
• Provide companionship and emotional support.
• Monitor and report changes in client condition to supervisor.
• Maintain a clean, safe home environment.
• Follow client care plans and agency policies.
• Document services provided and use EVV for scheduling.
• Communicate professionally with clients, families, and care team.
