Job summary

Gracious Hearts Homecare LLC is seeking dedicated CNAs to join our growing team in Illinois. You will provide compassionate personal care, mobility support, and daily living assistance to clients in their homes.

Responsibilities include following care plans, documenting care, transporting clients to appointments, and reporting changes to supervisors. A current CNA license, valid driver’s license, TB test, CPR/First Aid, and ability to lift 50+ lbs are required.

Qualifications

• Active CNA license in good standing.

Responsibilities

• Provide personal care assistance including bathing, dressing, grooming, toileting and mobility support.
• Assist with daily activities such as meal preparation, light housekeeping and companionship.
• Monitor and report changes in client conditions to the supervisor.
• Follow care plans and ensure clients receive proper attention based on needs.
• Assist with transportation to appointments, errands and other activities as needed.
• Maintain accurate documentation of care provided.
