The Opportunity

The Opportunity

The Care Manager will provide support, linkage, and coordination of services to children and youth ages 5 to 21 years of age who are diagnosed with a serious emotional impairment.

Responsibilities

  • Provide case management services through a minimum of 2 contacts a month
  • Complete and submit all assessments, care plans, progress notes and any other written work on time and in compliance with agency and regulatory standards.
  • Provide on-going assessment and documentation of child and family's strength and needs, progress towards achieving goals and efficacy of delivered services.
  • Maintain contact and hold interdisciplinary team meetings with providers and the family to maximize care.
  • Deliver and document all services using a strength- based and family driven approach.
  • Non-Medicaid only: Assist families with the C-YES process when determined a higher level of care is needed and maintain contact with C-YES and family until child is approved.
  • HHCM only: Ensure monthly billing is completed and submitted on time and monthly revenue expectations are being maintained.
  • Attend Division and program staff meetings on a monthly basis.
  • Additional tasks as assigned to support the operations of the program.

Qualifications

Bachelor's Degree required

Minimum Of 2 Years Of Related Experience

Intermediate Computer skills

Excellent verbal and written communication skills.

Valid NYS Driver's License

Additional Information

Schedule: F/T Monday-Friday

Salary: $22.63/hr

Location: Garden City
