Qualifications

• Clinical doctorate is required.
• Competence in delivery of healthcare services for adolescents, adults and geriatric patients is required.
• Licensed physician in the state of practice is required.
• Must be board certified or board eligible.
• BLS certification is required.
• Maintain a Controlled Substance license and Federal DEA, when applicable to their specialty.
