• Part-time practice in the Discharge Transition Clinic.
• Part-time inpatient rounding responsibilities with Sentara Hospital Medicine (SLH).
• Collaboration with hospitalists, PCPs, specialists, case management, health plan, and ambulatory partners.
• Participation in transition-of-care quality improvement efforts.
