Job summary

Avina Healthcare in Wisconsin seeks an Registered Nurse/Licensed Practical Nurse to provide high-quality nursing care to residents and supervise day-to-day nursing functions. You will coordinate all aspects of resident care with other disciplines and ensure compliance with federal and state regulations.

The role requires current RN or LPN licensure in this state, with responsibilities including medication administration, admissions/discharges, and documentation.

Qualifications

• Current licensed RN or LPN in this state.
• Provide quality nursing care to residents and supervise nursing assistants (CNAs).
• Coordinate resident care with other disciplines and follow regulations.

Responsibilities

• Assist in Admission/Discharge process
• Complete accident/incident reports and follow up investigations
• Transcribe physician orders and document resident information
• Learn and use Electronic Medical Record system; adapt to changes
• Prepare and administer medications as ordered by physician
• Participate in Care conferences and update care plans
• Maintain good rapport with all disciplines to meet resident needs
• Participate in continuing education programs
• Deal tactfully with residents, families, and staff
• Must hold current RN/LPN license in this state
• No mandatory overtime

Registered Nurse/Licensed Practical Nurse Job Summary

The Primary Purpose of this job is to provide Quality Nursing Care to the residents and to supervise the day-to-day nursing functions performed by the Nursing Assistants and coordinate all aspects of a resident's care with other disciplines in the center. Job duties are in accordance with the current Federal and State Regulations that govern the center.

Registered Nurse/Licensed Practical Nurse Responsibilities

• Assist in the Admission/Discharge process
• Complete accident/incident reports, follow up investigation process
• Transcribe Physician orders, document all resident information as necessary, notifying physician and family as needed. Review medication orders for completeness of information and accuracy
• Be willing to learn Electronic Medical Record system and be flexible to change as needed
• Prepare and administer medication as ordered by the physician
• Be familiar with the Care Planning Process and participate in Care conferences, updating as needed
• Develop and maintain a good working rapport with all disciplines to ensure that all aspects of resident needs can be adequately maintained
• Be willing to participate in continuing education programs
• Must be currently Licensed to practice as an RN/LPN in this state
• Should be able to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public
• Required licenses or certifications:
  Registered Nursing/Licensed Practical Nurse
• No mandatory overtime
