Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our compassionate care team. This role supports residents in private homes, skilled nursing facilities, and assisted living facilities with essential ADLs, mobility, and comfort as part of our hospice program.

Ideal candidates will hold a NJ CHHA license, have at least one year of hospice experience, and possess dependable transportation to visit multiple patient sites.

Qualifications

• HHA license in NJ required.
• Hospice experience preferred.
• Minimum 1 year of hospice work experience.

Responsibilities

• Assist with prescribed exercises and ROM as instructed by Hospice Nurse.
• Provide assistance in activities of daily living including grooming and personal care.
• Assist with toileting, transfers, and positioning.
• Administer medications as directed and report changes to the Hospice Nurse.
• Assist with light housekeeping and meal feeding.
• Take temperature, pulse, and respiration under supervision and report findings.
• Communicate changes in patient condition to nursing staff and document appropriately.
