Job summary

Swan Hospice in New Jersey seeks a certified Home Health Aide (CHHA) with hospice experience to join our caring team. You will provide compassionate in-home care to patients in private homes and facilities, supporting dignity and comfort.

Responsibilities include assisting with ADLs, ROM exercises, medication reminders, meal support, documentation to the Hospice Nurse, and coordinating with facility staff as needed. A NJ CHHA license is required, and dependable transportation is essential.

Qualifications

• Understands hospice philosophy.

Responsibilities

• Assists with prescribed exercises and ROM as taught by Hospice Nurse.
• Assists with toileting, transfers, and positioning of bed-bound residents.
• Provides or assists with ADL activities incl. personal care.
• Reminds residents to take medications per regulatory guidance.
• Light housekeeping of resident’s unit and laundry as needed.
• Feeds and assists with meals.
• Measures temperature, pulse, and respiration under supervision.
• Reports changes to the Hospice Nurse and documents details.
