Position Details

THIS POSITION IS A 3-MONTH CONTRACT STARTING April 2025 AND ENDING JULY 2025, WITH POSSIBILITY FOR EXTENSION OR MANAGEMENT DEVELOPMENT

Night shifts are M-F 11P-7A. Weekend shifts are Sa-Su 7A-7P, 7P-7A.

Second, third, and weekend shifts are encouraged to apply.

REQUIREMENTS: MUST BE AN RN, LPN, LCSW, CASE MANAGER, OR HAVE OTHER ILLINOIS CERTIFICATION/LICENSURE

Job Summary

Our case management team is seeking multiple compassionate and professional Emergency Department Discharge Planners to work within a local hospital's emergency room (ER). The ER Liaison will identify and engage eligible accident patients upon entry, provide immediate guidance on available resources, and facilitate initial steps of their care. This role focuses on the ER but involves collaboration with our external case management team for patient care and legal referrals.

Please note: This is a third-party role, not hospital employment, requiring nights, overnights, holidays, and weekends. Candidates interested in working all shifts are encouraged to apply.

Key Responsibilities
  1. Patient Identification and Engagement: Identify eligible accident patients in the ER, initiate contact post-stabilization, and inform them about follow-up care services.
  2. Cross-Department Collaboration: Work with hospital care teams to ensure smooth handover of patient information for ongoing external care, including legal support.
  3. Educational Support: Provide resources and information on next steps, follow-up appointments, specialists, and legal assistance.
  4. Documentation and Compliance: Record all interactions accurately, ensuring compliance with hospital policies, privacy laws, and confidentiality standards.

- Work closely with ER staff and case management to integrate workflows and ensure effective patient handoff.
