Qualifications

• Current Ohio RN license is required.

Responsibilities

• Provide direct patient care including assessments, medications, treatments, and wound care.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Monitor and report patient progress and escalate concerns as needed.
• Communicate with patients, families and staff for seamless care transitions.
• Maintain accurate patient records per regulatory standards.
