This role delivers direct patient care, medication administration, vital sign checks, and assistance with daily living activities within a warm, community-focused setting.

Qualifications

• Current RN license and certification in Pennsylvania.
• Current CPR certification.
• Ability to lift and transfer patients safely.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct patient care including administering medications and monitoring vital signs.
• Develop and implement individualized care plans with the interdisciplinary team.
• Monitor and report changes in resident conditions to ensure timely interventions.
• Maintain accurate and up-to-date records of resident care and treatment.
• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care.
