Qualifications

Responsibilities

• Collects relevant patient data and analyzes assessment findings to guide care.
• Develops care plans and prescribes interventions to achieve outcomes.
• Implements plans, coordinates care delivery, and ensures safe environment.
• Evaluates progress toward outcomes and adjusts plans as needed.
• Collaborates with patient, family, and healthcare team to provide compassionate care.
• Provides education to patients and families and supports team learning.
• Participates in discharge planning to ensure continuity of care.
