Zusammenfassung

Aliya Healthcare is seeking a dedicated Registered Nurse or Licensed Practical Nurse (LPN) to join our skilled nursing facility in Chicago. You will provide total nursing care on the assigned unit during the overnight shift and supervise team members to ensure high-quality resident care.

Responsibilities include developing care plans with the healthcare team, administering medications, educating residents and families, and maintaining safety and compliance with facility policies.

Qualifikationen

• Graduation from an accredited nursing education program.
• Active RN or LPN license with Illinois Department of Professional Regulations.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Must be able to work with diverse resident populations, including elderly and mentally ill individuals.
• Physically capable of performing nursing duties with or without accommodations.

Aufgaben

• Provide total nursing care to residents on the assigned unit during the shift as a Registered Nurse or LPN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
