Responsibilities

• Provide prescribed medical treatment and personal care to residents in Long-Term Care.
• Assess, plan, implement, and evaluate resident care using the nursing process.
• Complete admission assessments and home med reconciliation.
• Document and prioritize resident data across physical, psychosocial, spiritual, educational, and discharge needs.
• Communicate with residents, families, physicians, and health team members to ensure continuity of care.
