Qualifications

• BLS Provider within 1 Month (30 days) of hire date or job transfer date required.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire required.

Responsibilities

• Deliver direct patient care by implementing and adapting individualized treatment plans to promote safety and optimal healing.
• Coordinate multidisciplinary care teams to align around unified patient goals.
• Monitor patient status in real time by tracking condition changes and communicating updates promptly.
• Streamline the discharge transition by assessing ongoing patient needs and collaborating with care teams to support a safe return home.
