Qualifications

• Associate's degree in Nursing required.
• 1+ year of acute hospital experience preferred.
• 1+ 12-lead EKG experience required.

Responsibilities

• Conduct comprehensive clinical assessments of patients’ physical, mental, and emotional health.
• Develop and implement individualized care plans based on assessments.
• Coordinate multidisciplinary care among physicians, nurses, social workers, and therapists.
• Monitor progress and adjust care plans as needed.
• Document all assessments, plans, interventions, and outcomes in medical records.
• Administer medications and treatments following protocols.
• Educate patients and families about diagnoses and care options.
• Supervise nursing and support staff and evaluate performance.
• Plan and coordinate discharge to ensure smooth transition home or to another facility.
