Job summary

Responsibilities

• Assess patient clinical and psychosocial needs and apply standards of practice
• Partner with patients and families to develop and evaluate a plan of care
• Facilitate patient throughput and transitions to ensure quality care
• Coordinate and deliver individualized patient education and discharge planning
• Complete clinical and quality documentation per RHS standards
• Delegate and prioritize tasks to support care and safety
