Responsibilities

• Assess patient clinical and psychosocial needs per standards of practice
• Develop, implement and evaluate individualized care plans with patient/family
• Coordinate with interdisciplinary team for care transitions
• Educate patient and family on discharge plans and self-care
• Delegate and supervise non-professional staff to ensure safe patient care
• Document thoroughly per RHS standards and regulatory requirements
