Job summary

Qualifications

• Hold a valid/unencumbered license to practice (state-appropriate).
• Reliable transportation to patient homes.
• Provide TB results or chest x-ray as required.
• Provide CPR certification per agency policy.
• Be flexible, organized and exercise independent judgment.

Responsibilities

• Performs treatments and medication administration as per care plan.
• Collaborates with patient, physician and health care team.
• Identifies concerns with critical thinking skills.
• Completes all clinical documentation accurately and timely.
• Reports on time and as scheduled to complete work within the designated time.
• Follows all policies in Employee Handbook.
