Job summary

Advanced Health Care of Glendale is seeking an experienced registered nurse or licensed practical nurse to provide comprehensive nursing care in a skilled nursing facility. The role includes assessments, care plan development, and collaboration with physicians, residents, and families.

Qualifications

• Must hold a valid RN or LPN license in the state of employment.
• CPR certification, AZ Fingerprint Card and TB test required.
• Minimum of 1 year nursing experience, ideally in skilled nursing facility.

Responsibilities

• Assess residents' health status including vital signs, histories, and symptoms.
• Collaborate with residents, families, physicians and team members on goals of care and care plans.
• Regularly reassess needs and adjust care plans for continuity and responsiveness.
