Job summary

Stonecreek in Paducah, KY is seeking a Night shift LPN/RN to join our 90-bed community. We provide quarterly raises, monthly staff events, comprehensive benefits, tuition reimbursement, a 401K, shift differentials, DailyPay, and PTO accruals for full-time staff.

Core responsibilities include bedside care, medication administration, and care plan coordination. Requirements include a current LPN/RN license and at least one year of LTC experience; MatrixCare knowledge is a plus.

Qualifications

• Current LPN/RN license in state.
• At least one year of LPN or RN experience in a Long-term Care setting preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consults and coordinates with healthcare team to assess and plan patient care.
• Administers medications and records administration per policy.
• Monitors vital signs and maintains patient records.
• Provides bedside care and supports discharge planning.
