Job summary

MJHS seeks a licensed practical nurse to join the Menorah Center for Rehabilitation & Nursing Care team. The LPN will provide nursing care to residents in a long‑term care setting under the supervision of the Nurse Manager, Nurse Coordinator and/or RN.

You will help with assessments, care plans, medications, and patient comfort while documenting changes and communicating with the supervising staff. The facility emphasizes compassionate care, diversity, and a collaborative team environment.

Qualifications

• Certificate from an accredited LPN program.
• NYS LPN license required.
• Good clinical patient care skills.
• Valid BCLS.

Responsibilities

• Deliver nursing care to residents/patients in the long‑term care facility under supervision of the Nurse Manager, Nurse Coordinator and/or RN.
• Assist with routine assessments, medication administration and daily care plans as directed.
• Document care and report changes in residents' status to the supervising staff.
