Job summary

MJHS, in collaboration with the Menorah Center for Rehabilitation & Nursing Care, is seeking an LPN to deliver nursing care to residents in a long-term care setting under supervision by the Nurse Manager, Nurse Coordinator and/or RN.

Responsibilities include providing patient-centered care, collaborating with the care team, and maintaining high-quality standards. Eligibility requires a NYS LPN license and a certificate from an accredited LPN program, plus valid BCLS.

Qualifications

• Certificate from an accredited Licensed Practical Nurse Program.
• NY LPN license required.
• Good clinical patient care skills.
• Valid BCLS.

Responsibilities

• Deliver nursing care to residents/patients under supervision of the Nurse Manager, Nurse Coordinator and/or RN.
• Assist with assessments and care planning as part of the care team.
• Monitor patient status and respond to changes in condition.
