Job summary

Western Tidewater Community Service Board is seeking a License Practical Nurse for Windsor, VA, to join our CCSU facility. The role supports mental health and residential services, with a full-time 9:00 pm–7:00 am shift and rotating weekends, plus a part-time option two nights weekly.

Responsibilities include implementing care plans, conducting health assessments, medication administration, and ensuring safety and compliance, with VA LPN licensure required and a valid driver's license.

Qualifications

• Graduation from an accredited LPN program.
• Current, unrestricted LPN license in Virginia or a Nurse Licensure Compact state.
• Minimum of 1 year LPN experience in a mental health program or residential setting.
• Valid driver’s license with acceptable DMV record.
• Proficiency in Microsoft Office.
• Preferred: experience with EMR and Medicaid/Medicare regulations.

Responsibilities

• Implement and maintain Individual Service Plans for consumers.
• Conduct ongoing physical and mental health assessments.
• Maintain accurate consumer records and monitor safety.
• Supervise and oversee individual medical treatment programs.
• Administer and deliver prescribed medications as needed.
• Educate consumers on medication compliance and potential side effects.
• Assess vital signs, height, weight, and monitor medication compliance.
• Complete prior authorizations and assist with psychiatric medication needs.
• Ensure compliance with agency policies and relevant regulations.
