Job summary

Apploi is seeking a Licensed Practical Nurse (LPN) to care for residents in our Skilled Unit (SNF) and Long Term Care Unit. The role includes medication administration, including IVs, vital sign checks, and patient outcomes monitoring.

The LPN will provide resident and family education, coordinate with physicians, NPs and other health care professionals, and supervise CNAs. Current LPN license in good standing is required; SNF/long-term care experience is preferred.

Qualifications

• Current LPN license in good standing.
• Experience in SNF/long-term care preferred.
• Ability to administer medications and IVs.

Responsibilities

• Provide care for residents in the Skilled Unit (SNF) and Long Term Care Unit.
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
