Signature HealthCARE in Lexington, Kentucky is seeking a part-time Licensed Practical Nurse (LPN) for night shift 7:00 PM to 7:00 AM. Join a team focused on compassionate care in a 98-bed skilled nursing setting at Mayfair Manor.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity and safety.
• Supervise and direct nursing assistants and medication aides during the shift as per policy.
• Monitor residents' physical and emotional status and notify physician of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
