Job summary

Avalon Health Care Inc. is seeking a dependable LPN to join our Honolulu team, providing patient-centered care under supervision during the 11:00 pm–7:00 am shift. Placement within wage range is based on experience and qualifications.

Qualifications

• Must hold and maintain a current license to practice as a Licensed Practical/Vocational Nurse in Hawaii.
• CPR or BLS certification required.
• Ongoing CE credits pursued for professional development.
• Demonstrates working knowledge of infection control principles and practices.
• Excellent communication with staff, residents and families.
• Basic computer skills and ability to learn company software.

Responsibilities

• Under RN/physician supervision, participate in development and implementation of resident care plans.
• Deliver nursing care within state nursing practice guidelines and facility policies.
• Document care activities and monitor resident status and health needs.
