Job summary

Savannah post acute in Savannah, GA seeks a Licensed Practical Nurse (LPN) for the 7pm-7am shift to provide direct nursing care under RN supervision. You will administer medications, monitor residents, and support safe, compassionate care in a skilled nursing setting.

You will collaborate with physicians, CNAs, and the interdisciplinary team, document care accurately, and ensure HIPAA/compliance with facility policies. New grads considered with orientation; BLS/CPR preferred.

Qualifications

• Active, current LPN or LVN license in the state of employment.
• Graduate of an accredited practical or vocational nursing program.
• BLS/CPR certification preferred.
• Prior experience in skilled nursing/rehabilitation or healthcare setting preferred.
• New graduates may be considered with orientation and training.
• Strong clinical skills and effective communication.
• Ability to prioritize and manage time; maintain documentation.
• Basic computer skills; familiarity with electronic health records (EHR).

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Observe residents for changes and report findings promptly.
• Assist with admission, discharge, and transfer, including documentation.
• Perform wound care, catheter care, specimen collection within LPN scope.
• Provide routine nursing care to support residents' comfort, safety, dignity.
• Document all care and observations accurately in records.
• Communicate effectively with residents, families, and the team.
• Participate in care planning and implement individualized care plans.
• Supervise CNAs and other assigned staff.
• Ensure clear shift-to-shift communication through documentation.
