Job summary

Epic Healthcare in Pennsylvania is seeking a Licensed Practical Nurse (LPN) for overnight hours from 11pm to 7am to provide skilled nursing care to residents and patients in a long-term care setting.

The role emphasizes medication administration, patient assessment, wound care, and adherence to HIPAA and infection-control standards. You will document in EMR/EHR systems (Epic or Cerner) and collaborate with the care team to support discharge planning and dementia care.

Qualifications

• . Valid LPN license in good standing.
• . Experience in long-term care or skilled nursing facilities.
• . Familiarity with EMR/EHR documentation.
• . Knowledge of physiology, medical terminology, and diagnostics.

Responsibilities

• . Overnight care delivering nursing services including vital signs monitoring, medication administration, and wound care.
• . Conduct patient assessments and document findings in EHRs.
• . Manage IV infusions and catheter care with aseptic technique.
• . Assist with ADLs, including tube feeding, mobility support and hygiene.
• . Respond to emergencies using airway management and BLS as needed.
• . Collaborate with interdisciplinary teams for discharge planning and dementia care.
• . Ensure HIPAA compliance and maintain patient confidentiality.
