Alpine Rehabilitation and Nursing Center in Xenia, OH is seeking a Licensed Practical Nurse (LPN) for a full-time night shift (6p–6a). You will provide direct patient care, administer medications, take vital signs, and document care, collaborating with the interdisciplinary team to implement individualized care plans.

Qualifications

• Current LPN licensure in the state of Ohio.
• Minimum of 0 years of experience as an LPN.

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct patient assessments, report findings, and implement interventions as necessary
• Collaborate with caregivers to ensure patient needs are met and reported
• Maintains accurate and timely records of patient care, including medications, treatments, and test results
• Participate in ongoing education and training to stay current with industry standards and best practices
• Supports a positive and respectful environment for patients, families, and fellow staff members
