Job summary

Springdale Health and Rehabilitation, under NHS Management, LLC, seeks a skilled Licensed Practical Nurse (LPN) for night shift (11PM–7AM). You will provide direct nursing care, assist with rehabilitation plans, and collaborate with RNs and physicians to uphold high standards of care in a long-term care setting.

The role requires a current LPN license, strong clinical skills, and CPR/BLS certification. Benefits include health insurance, PTO, and a $10,000 sign-on bonus.

Qualifications

• Current and valid LPN license.
• Strong clinical skills including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-based environment.
• Compassionate, patient-focused approach to care.
• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
• Assist with activities of daily living (ADLs), including bathing and feeding.
• Collaborate with rehabilitation team to support residents’ recovery goals.
• Maintain accurate documentation in accordance with facility policies and regulations.
• Provide emotional support and education to residents and families.
• Ensure a safe and clean environment adhering to infection control protocols.
