Job summary

Spring Health in New York, NY is seeking independently licensed mental health professionals to join the Care Navigation team for overnight coverage. Responsibilities include crisis support, referrals, documentation, and coordinating care with internal and external providers.

The role requires a strong clinical background and the ability to work independently during non-peak hours. Eligible candidates hold LCSW/LMFT/LPC or psychology licensure, with 2+ years of direct clinical care.

Qualifications

• Licensed mental health professional (LCSW/LMFT/LPC/psychologist).
• Minimum of 2+ years direct clinical care experience.
• Expertise in crisis assessment and safety planning.
• Knowledge of crisis support, CBT, motivational interviewing, mindfulness, and solution-focused interventions.
• Fully engaged with minimal interruptions for the duration of shift.

Responsibilities

• Provide in-the-moment clinical support and referrals via a clinical phone queue.
• Use active listening, motivational interviewing, and solution-focused approaches to engage with members.
• Assess risks and provide crisis support with follow-up to ensure safety and connection to care.
• Coordinate action plans and connect to Spring Health therapists, coaches, physicians, or external resources.
• Provide after-hours support to members and leaders, and connect with the Global team.
• Complete professional documentation and communicate with members and providers via email.
• Follow care management outreach protocols and identify barriers to care.
• Coordinate with Spring Health care providers and the Provider Operations team.
• Collaborate with Care Support, Benefits/Billing, and Product for a smooth member experience.
• Participate in staff meetings, case consultations, trainings, onboarding, and mentoring.
