Responsibilities

• Assess, plan, implement and evaluate care for a specific patient assignment.

• Document patient care per nursing documentation standards.
• Develop and implement comprehensive care plans based on initial assessments and input from the health care team.
• Provide education for patient and family and arrange discharge planning.
• Collaborate with other disciplines for optimal patient outcomes.
