Qualifications

Current, valid VA license/certification in good standing.
Minimum of 1+ year within the last 3 years of experience as a Licensed Practical Nurse.
Copy of Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
Copy of all current required certifications and compliance documents.

Responsibilities

Assess patients' health conditions and develop individualized care plans.
Administer medications and treatments as prescribed by physicians.
Monitor patient progress and document assessments, interventions, and outcomes.
Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care.
Educate patients and families on diagnoses, treatment plans, medications, and discharge instructions.
Maintain accurate and timely electronic medical records (EMR) and ensure infection control standards.
