Job summary

SSM Health St. Clare Hospital in Baraboo, WI is seeking a Regular full-time Nursing Assistant to join the 2 West Med Surg team.

You will work nights, 7pm–7am, with a 36-hour weekly schedule and every third weekend rotation, under the supervision of a registered nurse to deliver essential patient care. This role requires an active Wisconsin CNA certification and education equivalency for licensing; no prior experience is required.

Responsibilities

• Receives patient care assignment from the RN in charge and initiates delegated tasks based on demonstrated competence.
• Collects and documents data for admission assessments and re-assessments and provides data to the licensed nurse for analysis.
• Relays information via charting and communicates with team members.
• Informs the licensed nurse of changes in patient condition or needs.
• Assists with daily living activities and basic nursing tasks including vital signs and patient safety.
• Prepares patients for discharge and accompanies them when dismissed as indicated.
• Maintains supplies and equipment and assists with delegated clerical duties.
• Orients new patients and significant others to the unit.
• Operates equipment safely and may perform point-of-care testing according to policies.
