Rochester Regional Health is seeking a Certified Nursing Assistant at Unity Living Center in Rochester, NY. This role involves hands-on patient care in the Long Term Care department, with nights and weekend rotation.

The CNA collaborates with nursing staff to support care plans and ensure resident comfort. Responsibilities include assisting residents with activities, transporting to activities, documenting observations, and supporting families.

Qualifications

• Completion of NYS approved certified nursing assistant program
• Minimum age requirement: 18
• Registered Certified Nurse Aide in New York State
• Associates degree is preferred

Responsabilités

• Provide and/or assist the resident in activities necessary as per individualized care planning
• Transport to and from activities as needed
• Provide leisure activities as needed
• Perform evaluation and reporting
• Provide resident and family with emotional support
• Safe, clean, and safe resident environment maintenance
• Respond to resident requests in a timely manner
• Receive/give reports to team members/team leaders
