Qualifications

Responsibilities

• Assist residents with activities of daily living including ambulation, bathing, dressing, eating, toileting, personal hygiene
• Promptly report changes to Licensed Nurse
• Participate in admission, transfer, and discharge procedures
• Support care planning conferences and transport residents to activities
• Document data accurately and promptly in required forms
• Respond to resident call lights promptly
• Follow facility policies and procedures and identify safety hazards
• Initiate appropriate action during emergencies
