Qualifications

• High School Diploma or GED is required.
• One year preferred.
• One year Assisted Living facilities experience (long-term care or assisted living) is required for that unit.

Responsibilities

• Deliver high‑quality, patient‑centered care by using clinical experience to anticipate needs and promote comfort and dignity.
• Serve as a trusted partner to nurses and providers, sharing observations and supporting care plans.
• Guide safe mobility, ambulation, repositioning, and fall-prevention practices.
• Ensure accurate, timely documentation of vitals and patient status.
• Foster a calm, supportive environment for patients and families, offering reassurance and setting expectations.
• Provide assistance to patients with activities of daily living and maintain a safe, clean environment under RN supervision.
• For Assisted Living units: assist residents with daily living activities and report changes to the Charge Nurse.
