This full-time position involves ensuring safe and effective medication management, supporting clinical decision-making, and participating in multidisciplinary rounds. Candidates must have a Bachelor's Degree in Pharmacy or PharmD, Texas Pharmacist License, and a minimum of 2 years relevant experience. Qualifications

• Texas Pharmacist License (or must obtain within 90 days).
• Texas Pharmacist Preceptor Certification (within 180 days).
• ACPE IV Sterile Certification required.
• Basic Life Support (BLS) within 90 days.
• Advanced Cardiac Life Support (ACLS) required for inpatient roles within 6 months.
• Board of Pharmacy Specialties (BPS) Certification preferred.
• Minimum 2 years of progressive, relevant experience in outpatient/retail or inpatient settings.
• PGY1 residency highly preferred.

Responsibilities

• Ensure safe, effective, and optimal medication management across all dimensions of patient care.
• Interpret and evaluate medication orders and support clinical decision‑making.
• Perform key pharmacy functions: order entry, pharmacokinetics, IV-to-PO conversions, and therapeutic interchange.
• Participate in multidisciplinary rounds and provide direct patient care.
• Contribute to staff education and mentoring pharmacy interns, technicians, and residents.
