Job summary

Qualifications

• Requires a Master's degree in social work, psychology, nursing, or counseling.
• Licensing appropriate to the profession.
• Ability to assess behavioral health needs and risk.

Responsibilities

• Provide direct clinical services to patients, families and others.
• Perform mental health and chemical dependency evaluations; crisis intervention.
• Offer supportive counseling and referrals for additional services.
• Prepare documentation for services and maintain patient records.
• Obtain informed consents for admission to the program.
• Maintain confidentiality and discuss patient information only with appropriate personnel.
• Promote safe practices and uphold Trinity Health System standards.
• Performs other duties as assigned.
